That's the Lensbaby, Its basically a very simple lens stuck in the end of a flexible corrugated tube. The lens has a 'sweet spot' where things are in focus, and you can 'move' this sweet spot around the subject by flexing the flexible tube. You can also 'change' focus for near or far subjects by compressing/ extending the tube.

It's a very nice toy, but fairly tricky to use
