Me, I've come close on a few,

turning an element on with no water in the hlt = black blue element/smoke...luckily it didn't blow it still works to this day
turning heat exchanger on but not the recirc pump = rumbling noise from HE and boiling water spewing out from the top all over the bench
turning hose water on to plate heat exchanger but forgetting to attach hose to PHE outlet to drain = water everywhere

A brewday wouldn't be complete without one or two hiccups :grin:
