From my limited experience (3 AG brews so far bottled and one racked to keg), I have had good results with:
Bottling (for 23l batch) - put the required about of sugar (50-80g) into a sterilised vessel (saucepan/pyrex jug) with about 150ml water and bring to the boil. Cool, then add to the beer in the FV to be bottled. Stir very gently and leave for 30mins to 1 hour before bottling. Very easy and seems to work. Not had any problems with bottles not carbonating.
* Don't add finings when bottling as it can cause the yeast to be VERY unstable :x

Racking - Boil up the sugar solution as above, cool, and add to the keg before racking. Add finings too if desired.
