Tasting sheet

Does anyone have a PDF or word tasting sheet they use with all the flavors and a check box next to them? Kind of a cheat sheet to help remember flavors while sampling.

Check out bjcp.org > Competition Center, scroll down to the bottom, and the checklist scoresheet might be just what you’re looking for. Also the Beer Faults Troubleshooter is a companion guide for using the checklist score sheet.

So here is a first draft pulling from all the sites recommended. I don't really like it though it seems a little too check the box. I might do something like the lists for reference but leave it open more like the BJCP sheet.

Anyway maybe someone else will get some use from this.
