No head on IPA

A few weeks back I brewed my first IPA (see here: http://www.thehomebrewforum.co.uk/viewtopic.php?f=21&t=34222); I've listed the grain bill below. For some reason, it doesn't develop a head. It's been conditioning for 4 weeks, two of which were in a secondary FV and the last two have been in the bottled. The beer was batch primed with 80g of Sugar. I followed guides of how much grain to add to give head but it doesn't have seem to have worked. Any help would be appreciated.

Grain bill:
4.65kg Pale Malt
0.515kg Caramalt
0.175kg Crystal
0.170kg Flaked Barley
 
It should. Could be a case of cleaning residue on your bottles or even simply your glass?

I've tried two different glasses to eliminate that. I read somewhere that dish washer cleaned glasses can cause head retention to go down hill. Could the serving temperature have a bearing on it? The bottles are being stored in my garage which is around 8c.
