Lager fermenting at 12.5 degrees for 14 days - should I up the temp now?

I have had a lager that was fermenting with lager yeast, in the temp controlled fridge for 2 weeks, should I now raise the temp by say 2 degrees a day up till it reaches 21, then leave it for a few days? Do I then need to lager it in the primary for 3 weeks (at around 2 degrees) or can I chill and then bottle as a normal ale - and lager in the bottle?
Never brewed with Lager yeast before you see.....
 
I normally do as you suggest, raise the temperature gradually to about 18-20° for a couple of days then drop to 0° for lagering. Best to lager in bulk, before bottling imo.
