First extract brew boiling question

Ok so in the morning I'm going to create my first extract brew after doing some kits in recent months. I'm wanting to do some extract before going into AG. I've got what is going to be a daft question but I've looked around on here and in the books I've got and I can't see any reference to it.
I've got one of the extract kits from HBC The golden ale kit. There's a link that has a walk through to brew this, as with any in general anyway. The method being 3l x2 to steep and spare the grain and then pour into the boiler. And here is my question.
I've got a buffalo boiler, that I will have filled with 19l to which ill add the 6l from steeping, but am I adding the steeped wort into boiling water or am I adding it to cold water and start the boil from fresh which will chill the liquor?
It's going to be really obvious to everyone else I'm sure but I just want to sanity check.
 
Steep your speciality grains from the kit in the 6 litres and while that's steeping mix up your 2.5 ~ 3 Kg of DME with the 19 litres in the boiler. Get that heating and add the speciality grain wort when it's ready. Bring the whole lot up to the boil and start your hop timings from there....

I've made my brew this morning, I've got my OG of 1.046 but I've got a fatty looking sediment deposit in the test tube of about an inch, I think I can see the same in the fv of around 2 to 3 inches. Is that something to worry about or will it clear up?
