CO2 regulator issues

I went to connect a new gas bottle to my regulator ready for summer. When I turned the gas on, taps closed on the regulator the gas instantly started firing out of the PSV like a machine gun? Obviously it should hold the pressure but it isn't.
Any ideas on why this is happening and how it can be fixed? Has anyone had the same problem here?

Are you sure the taps were closed? Not as crazy as it sounds because the normal method of increasing flow on a regulator is to rotate the tap clockwise - i.e. the same way you'd normally rotate a sink tap/faucet to shut the flow off.
 
Firstly you don’t look old enough to be drinking.

They only time I’ve had a similar issue is when the gas supplier has a few rouge bottles in that had dip tubes in so I was dispensing liquid CO2. The reg froze up and the PRV’s were popping away merrily.
