Extract brew: Steeping the grains

Quick question after reading conflicting advice...

1. Soak the grains at 66degC for 30mins, twice then add the resulting wort to the pan, add DME etc
or
2. Put the grains in a bag and bung 'em in with the DME at 40degC and bring to the boil, removing them just before.
or
3. As 2. only remove before 75degC so as not to introduce 'tannins and the resulting off flavours'.

I was planning on doing 2. though I'm guessing I'd need to ensure it took at least 30mins to get from 40degC to boiling in order to get the most out of the grains?
 
ive got one of they brewpak boilers with the temp dial, i half fill it with cold water, turn it to the mash temp setting which holds it around 64 degrees, fire in my grains for 30 or 40 mins, take them out then add the dme, bring to boil. when i first brewed extract i put my grains in with the cold water and let it warm up to 64 degrees then took the grains out, it all ends up the same!
